We may use or disclose your health information for: Providing and coordinating chiropractic treatment, billing insurance companies and processing payments, managing and improving our practice, appointment reminders and treatment-related communications, legal requirements as permitted or required by law. Other uses or disclosures may require your written authorization.
 
Your rights: You may request access your medical records, request corrections, ask for confidential communications, request certain restrictions, and receive a copy of this notice.
